The New York State Department of Health Asthma Coordinator Patricia Waniewski came to visit the Suffolk County Department of Health to for the counties unveiling of the Asthma Demonstration Project. The Suffolk County Asthma Demonstration Project will reduce hospitalizations and emergency room (ER) visits for those with a diagnosis of asthma who receive their primary health care at one of the County’s network of health centers. This will be accomplished by: Provider education and performance improvement initiatives; in-home environmental assessment, health monitoring, and remediation; health education to asthmatics and their families; public health nursing in-home education; interdisciplinary team meetings; and, coordination of care between hospitals and health centers. |
